Pioneer Valley Memory Care Initiative (PVMCI)

Northampton Neighbors is committed to and actively engaged in making Northampton a truly Dementia Friendly community. PVMCI, a coalition of healthcare and community providers including Northampton Neighbors, serves those living with or caring for someone with memory loss, Alzheimer’s Disease and other dementias who reside in Amherst, Easthampton, Northampton, Florence, and Leeds.
Through community linkages, home visits, and telephonic support, PVMCI engages your strengths, overcomes barriers, and finds solutions to enable you or your loved one to remain at home and connected to your community.
PVMCI Provides
- Assignment to a Memory Care Specialist (MCS) who will be your main point of contact with the PVMCI program.
- Expert and comprehensive services to address memory loss, dementia and Alzheimer’s disease.
- Individualized care and services specific to your strengths, goals and needs.
- Support, education and creative solutions to address the challenges you may be experiencing.
- Enrollment in Northampton Neighbors, eligible for all NN services and programs as well the services of a PVMCI trained volunteer.
- Connection to community providers like Northampton Senior Services, Highland Valley Elder Services or the Alzheimer’s Association.
How to Participate
PVMCI is based out of the Cooley Dickinson Hospital (CDH) Geriatrics program and is led by Rebecca Starr, MD, Medical Director, Geriatrician. There is no cost to enroll. However, participation in PVMCI requires an assessment from Dr. Starr’s office.
